How your information is handled

Information we collect

We may collect contact information, NDIS and funding information, referral details, health and disability information, assessment findings, reports, communications and billing records. We collect only information that is reasonably necessary to provide and manage occupational therapy services.

How information is used

Information may be used to assess suitability, arrange services, complete the functional assessment, prepare reports and recommendations, communicate with authorised people, meet professional and legal obligations, manage accounts and improve service quality.

When information is shared

Personal information is shared only with consent, where it is necessary to provide the agreed service, or where disclosure is required or authorised by law. This can include sharing a completed report with the participant and people or organisations the participant has authorised.

Consent and authority to share information will be clarified during the referral and assessment process.

Storage and security

Reasonable administrative, technical and physical safeguards are used to protect personal information from misuse, interference, loss and unauthorised access, modification or disclosure. Please do not send clinical reports or sensitive documents to the general enquiry email address until a secure sharing method has been confirmed.

Access and correction

You may request access to personal information held about you or ask for it to be corrected if it is inaccurate, out of date, incomplete, irrelevant or misleading. Some legal or professional exceptions may apply.
